The 10% Problem: Making Biotech Decisions Count
Fewer than one in ten drugs entering Phase I reach approval, and among those that do, many still leave substantial commercial value unrealized in at least one major market because of decisions made years earlier. A large pharma company can absorb these losses across a portfolio. For a biotech CEO, however, each asset is a primary value driver and each decision is a draw on finite capital. While much of what drives failure is genuinely uncontrollable — biology, translation, target viability — a meaningful share of value leakage is decisional, concentrated in five control points where leadership shapes capital intensity, retained risk, and the credibility of the resulting evidence. The boundary between controllable and uncontrollable is also shifting: advances in computational biology, real-world data, biomarker science, and AI are making target validation, patient stratification, trial design, and evidence generation faster and more tractable, raising the standard for rigorous pre-commitment work. This session is a facilitated working conversation on how to make the next decision well across the five points where value is made or lost: indication and population, target product profile and pivotal commitments, trial design and evidence strategy, market preparation and access, and commercial readiness.
